Patient Data Management

The HSE West North-West Region is responsible for delivering Acute, Community and Public Health services in this catchment area. These services will increasingly be provided through Integrated Care, where the goal is that people can access the right care, at the rights time, in the right place, with the right team. Integrated Care will bring together services in hospital and non-hospital settings and this will be enabled by the sharing of personal data of patients / clients between different settings, so that joined up Integrated Care is delivered closer to home.

In essence, this involves sharing of personal data of people held in our information systems with staff in community and hospital settings, on a need to know basis, in order to deliver the most up to date information to Health Professionals in the provision of care to you.
